All current students, staff, and faculty are able to submit e-forms through Etrieve for most requests and procedures. Etrieve Forms is an online platform that allows students/faculty/staff to fill out, review, comment, approve, deny, and route forms as needed. This process is the most efficient way for current students, staff, and faculty to have requests processed by the Office of the Registrar.

Some forms are not yet available as an e-form, but are still accessible in a pdf format.

Navigate to our Academic Forms webpage for the full list of forms available for current students, staff, and faculty.
